The Advisory Councils Transformed (ACT) Framework
Reimagining Engagement | Expanding Impact | Including Every Voice
The ACT Framework is a distributed model of engagement of patients, family caregivers, and communities in health system operations to improve outcomes.
It bridges the gap between patients, family caregivers, communities, and healthcare systems by turning people’s lived experience into actionable change.
Is your healthcare organization committed to partnering with people with lived-experience, but struggling to IMPACTFULLY engage a Patient and Family Advisory Council (PFAC) or similar group?
You're not alone — and you're not behind.
ACT is here to help.

Why did we create
The ACT Framework?
Traditional Advisory Councils often lack diverse representation and can have limited influence within an organization. It can feel like a burden instead of an enriching, collaborative experience. Oftentimes the participation of Patient Family Partners (PFPs) is siloed and limited making it harder to get important feedback from different voices.
The ACT Framework offers a flexible engagement approach that increases impact while optimizing staffing resources.
What our partners are saying about ACT…
"PFCCpartners have been just phenomenal. We cannot have done this without them. We've been together since 2018. So it's just a testament of the longevity of our relationship, our working relationship, how much we value them and how much we lean on them to continue to help us navigate."
— Miguel Renteria, Director of Patient Experience, Los Angeles County Health Services
"Now, because of what Libby and team have really demonstrated for me, we have to have patients and family members at the design table because we just aren't going to know what the challenges are without them."
— Kim Werkmeister, Senior Vice President, Improvement and Implementation, Convergence Health
